1. Identify the removed material or the material to which access has been disabled, and specify its location before removal or disabling.

2. You must state, under penalty of perjury, that you have a good faith belief that a mistake or misidentification led to the removal or disabling of the material.

3. You must provide your name, address, and telephone number, and state that you consent to the jurisdiction of the Federal District Court for the judicial district where your address is located.If you live outside of Pakistan, you must consent to the jurisdiction of any judicial district where the service provider is located. Additionally, you must accept service of process from the person who provided notification under DMCA subsection (c)(1)(C) or that person’s agent.
